UT Dental Branch Employee Relations Committee launches new   

 "Star Performer Award " 
 

Employees are the main ingredient in the recipe for success at The University of Texas Dental Branch at Houston. Whether it's a simple task or a complex project, teamwork among employees makes it easier to achieve successful results. 

That’s why the Dental Branch wants to reward those “shining stars” among us with a new Star Performer Award! Star employees at the UT Dental Branch create a synergistic effect that lightens the load and keeps all eyes on the prize.

To be eligible for the “Star Performer Award,” an employee must be:

  • Nominated by fellow employees and/or clients.
  • Selected on a quarterly basis from a committee of their peers
  • Employed full-time (100%) for one year.

Nomination Schedule
The Dental Branch Star Performer Award will be given to one person each quarter. The quarters are as follows:

  • 1st Quarter: May, June and July
  • 2nd Quarter: August, September and October
  • 3rd Quarter: November, December and January
  • 4th Quarter: February, March and April

The nomination forms for each quarter must be submitted by the 15th of the last month in that particular quarter (example, July 15th would be the date for the 1st quarter).  The committee would then meet in the last two weeks of each quarter to select a winner.

Star Performer Award honorees will receive:

  • a departmental pizza party
  • a gift card
  • one day of administrative leave (Taken at an agreed upon time between the recipient and  their supervisor)
  • a desk award
  • a photograph on the Dental Branch Web site

To nominate someone for the Dental Branch Star Performer Award, complete the following form and include any additional supporting materials.  Criteria and examples of excellence must refer to experiences within the past 11 months.

Return the signed nomination form to the chair of the selection committee (see chairperson’s contact information below). A signature and contact information is required on all nomination forms.

The Dental Branch Star Performer Selection Committee reviews all nominations quarterly.  If your nominee is not selected within the current quarter, all nominations will be kept on file for consideration for one year from the date of submission.  Any nominee not selected within one year, requires a new and/or updated nomination for consideration.

Previous award winners are not eligible for consideration for two years after receiving an award. Awards must be redeemed within one year or the award is forfeited.

Section 1 - Caring: Describe how the nominee illustrates that he or she cares for others, for co-workers, for clients and for the UT Dental Branch or UT Health Science Center at Houston community.  An example of caring might include the employee’s dedication to helping someone in need.
