NEW FACULTY ORIENTATION CHECKLIST

ALL FACULTY:

Approved by:

               1. Participate in New Faculty Orientation by Office of Educational Research and Professional Development
               2. Develop individualized Faculty Development Plan and present to department chair.
               3. Participate in Transforming Teaching Workshop/s offered by the Office of Educational Research and Professional Development.
               4. Peer Review of Teaching (should occur within the first semester of employment).

DEPARTMENT ORIENTATION:

              1. Meet with approved department chairs, division heads, section heads for introduction and orientation to departments.
              2. Tour departmental office and meet support staff
______  3. Administrative Assistant initiates the following:
             a. new employee information sheet
             b. appointment for PeopleWorks orientation
             c. memo to receive office and lab keys (if applicable)
             d. if necessary, contact UT Police for programming of identification badge for Dental Branch building and Learning Resource Center (LRC) access.
             e. email request form
             f.  request long distance telephone code (see instructional sheet or video for operations)
             g.  Xerox codes for copying
             h. vendor codes
             i. instructions for completing time sheets
             j. radiation approvals 
             k.  animal approvals
             l.   DNA approvals
             m. biohazard approvals
             n.  human subjects approvals
             o. order business cards

CLINICAL ONLY:

               1. Application for Clinical Privileges completed and submitted to department chair.
               2. Request and receive temporary Clinical Privileges - requires a completed Credentials Application. *(Associate Dean for Patient Care)
               3. Scheduling and completes Clinical Services orientation including *(Associate Dean for Patient Care of designee): including Safety and Infection Control and Medical Emergency Management)
               4. Complete Patient Services orientation.
               2. Complete CIS Training (if applicable)
